Understanding Your Baylor Scott & White Employee Benefits

The Baylor Scott & White Employee Plan includes four primary plan types:

  • SEQA Plan
  • EQA Plan
  • PPO Plan
  • HDHP Plan

All four plans include eligible preventive care at no cost when plan requirements are met.

The SEQA and EQA plans use the BSW Premier HMO network as their first level of coverage. The PPO and HDHP plans may have different deductibles and cost-sharing amounts based on the provider’s network tier.

Coverage for IV services depends on your eligibility, medical needs, diagnosis, provider network, remaining deductible, and the treatment performed. Not every hydration or wellness service is covered by health insurance.

SEQA and EQA Copays

For participating Tier 1 providers, the available plan information lists the following visit costs:

SEQA Plan

  • Primary care visit: $10 copay
  • Specialist visit: $40 copay

EQA Plan

  • Primary care visit: $30 copay
  • Specialist visit: $50 copay

These copays do not automatically apply to mobile IV services. Your responsibility depends on how the service is classified, whether it is medically necessary, the provider’s network status, and the benefits available under your individual plan.

PPO and HDHP members may be responsible for a deductible, copay, coinsurance, non-covered service charge, or full payment for services that do not qualify for insurance coverage.

Mobile IV Care Brought to You

IVTherapy2go allows eligible patients to receive IV care without making an additional trip to a medical facility. A healthcare professional will review your medical information to determine whether mobile IV treatment is appropriate and safe for you.

Depending on your medical needs and the provider’s clinical judgment, care may include hydration or other prescribed support. Every patient is evaluated individually, and treatment recommendations may differ.

The medical professional may determine that mobile IV care is not appropriate based on your symptoms, health history, vital signs, or the severity of your condition. When needed, you may be directed to an urgent care center, primary care office, or emergency room.

Mobile IV therapy is not a replacement for emergency medical care. Call 911 or visit the nearest emergency room for severe dehydration, chest pain, significant difficulty breathing, confusion, loss of consciousness, signs of a stroke, uncontrolled bleeding, or another serious condition.

What Will You Pay for Mobile IV Care?

Your financial responsibility depends on your plan, provider network, remaining deductible, medical necessity, and the services provided during your appointment.

IVTherapy2go can collect your insurance information and request benefit verification, but Baylor Scott & White Health Plan makes the final determination regarding eligibility, medical necessity, covered services, and claim payment. A benefit quote or cost estimate does not guarantee that the requested treatment will be covered.

Prior Authorization and Claim Information

Some medically necessary services may require prior authorization. Healthcare providers can use the BSW Health Plan provider portal, call (866) 384-3488, or fax the request to (800) 626-3042.

Electronic claims may be submitted through Availity using Payor ID 94999. Mailed claims should be sent to:

Patients generally do not need to submit claims themselves when the healthcare provider files the claim.
